Establishing Benchmarks for Clinical Document Data Quality

Clinical documents standards, such as the Consolidated Clinical Document Architecture (C-CDA), have grown significantly in adoption and maturity over the past decade. Information exchange during care transitions routinely use these documents today. Recently, health plans and other stakeholders have begun to incorporate clinical documents into quality measurement programs for value-based care. However, the reliability of structured data in clinical documents has not been well profiled across a variety of organizations and electronic health record (EHR) technologies. This research examines data quality among clinical documents from HealtheConnections, a large health information exchange in New York State.
